Stonebridge Pharmacy, a division of Stonebridge Medical, was established to provide a level of personalized service beyond the capabilities of the giant corporations. We are an independently owned and operated corporation with hands-on management. Our principals have a combined total of more than 35 years experience in the long term care industry. CLINICAL PHARMACISTS
Our commitment is to provide professional support to your medical and nursing departments with Stonebridge's qualified staff of clinical pharmacists. Our goal is to decrease hospitalizations, avoid drug misadventures and confirm that all residents receive optimal drug therapy. Stonebridge will work with your pharmacy and therapeutics committee, discussing ways to lower costs as well as evaluating the quality of drug therapy. It's the nearest thing to having a clinical pharmacist on your staff.

IN-SERVICE EDUCATION AND ON SITE PHARMACIST
Many pharmacies may offer the above services, but we go one step further. Stonebridge is committed to being more than a vendor resource because we have people on the other end of the phone giving value-added service all the time. Fully qualified, our clinical pharmacists have extensive experience with medical rounds combined with the ability to communicate clearly with your staff, the residents and family members. You'll get to know them personally and feel comfortable in trusting their recommendations. Stonebridge is thoroughly equipped with staff and material to provide the inservice training that would be beneficial to you and your staff. A clinical pharmacist will visit your facility weekly, to interface with multidisciplinary health care teams and to talk with staff and residents alike.
